Why the consistency of sleep timing predicts mortality risk more strongly than sleep duration — and what that means for working with your clients.
Almost every sleep recommendation revolves around a number. Seven to nine hours. Duration is treated as the decisive measure. A large analysis of more than 60,000 people calls exactly that focus into question. The findings are precise enough to change how we assess sleep in health optimization.
What the study measured
Windred and colleagues from Monash University and Harvard Medical School analyzed data from the UK Biobank: 60,977 individuals, over 10 million hours of objectively recorded sleep-wake patterns via actigraphy. Not self-reported questionnaires, but measured movement data.
From this data they calculated the Sleep Regularity Index (SRI): a measure of how similar a person's sleep timing is from one day to the next. They then compared what better predicts mortality risk — regularity or duration.
The result is unambiguous.
The numbers that matter
People with the most regular sleep timing had a substantially lower risk compared to those with the most irregular patterns — adjusted for age, sex, ethnicity, socioeconomic factors, and lifestyle and health variables:
- All-cause mortality: 20 to 48 percent lower
- Cancer mortality: 16 to 39 percent lower
- Cardiometabolic mortality: 22 to 57 percent lower
The key finding: sleep regularity predicted all-cause mortality more accurately than sleep duration. Irregular sleepers carry a risk that sufficient hours alone cannot offset.

Why the body rewards rhythm
The body runs on an internal clock. Circadian rhythmicity governs hormone release, body temperature, metabolism, immune function, and blood pressure across the day — in a coordinated, not isolated, way. Regular sleep timing keeps this clock on track. Constantly shifting times throw it off.
This state is comparable to what is known as social jetlag: the body is living in two different time zones simultaneously — biological and social. That has measurable consequences for inflammatory load, blood glucose regulation, and cardiovascular parameters.
A simple, evidence-based anchor: wake up at the same time each morning and seek daylight early. Morning light is the strongest signal that recalibrates the internal clock each day. A tolerance window of around 30 minutes is considered unproblematic. The occasional exception is too.
What this study can — and cannot — tell us
Honest context matters here.
This is an observational study. It shows a strong, cleanly adjusted association — but not proof of causality. Reverse causality is plausible: an emerging illness could make sleep more irregular while also increasing mortality risk. The cohort was also on average over 60 years old, which limits how directly the findings transfer to younger client groups.
Even so, the message holds. Sleep regularity is a free, realistically actionable lever. It is also one of the few that can be made visible with objective data.
What this means in practice
This is where the difference between a lifestyle tip and a measurable intervention becomes clear.
Holistic: Sleep regularity reaches far beyond sleep itself. It touches hormone profiles, blood glucose regulation, blood pressure, inflammatory markers, and mood regulation. The principle is the same one we see across other domains — whether gut microbiome, training adaptation, or behavior: consistency over time outperforms the single large effort. The body rewards rhythm.
Precise: Regularity can be measured. Actigraphy and wearable data make sleep-wake consistency visible. HRV and resting heart rate reflect actual recovery quality. A vague intention becomes a trajectory you can read, interpret, and act on as a health professional.
